The Kages set and enforce policies and new rules. They have a counsel of advisors that can try to grasp and fight with them for power, but they also have the ability to ignore it anyways. This counsel usually appoints and recommends the next kage too, and after that, they have to garner the approval of Jonin by vote. They also have their own bosses, called Daimyo, that monitor the entire country. They're disconnected with the village, but they do check in on affairs, approve treaties and wars, and nominate kages. They aren't very powerful besides that, as most kages override them anyways and make decisions and just deal with them later.
And yeah. It's discontinued now but I guess it was some twisted survival of the fittest. The recent kage has modeled the graduation system on a series of exams based on knowledge, demonstration of skills, and overall attitude.
